RESUME
Introduction : Le rein en fer à cheval est une fusion congénitale des reins. Il est souvent asymptomatique mais peut être le siège de complications urologiques. Nous Rapportons un cas rare d’abcès rénal localisé sur l’isthme d’un rein en fer à cheval.
Observation : une femme de 42 ans présentait une douleur chronique de l’hypochondre droit. L’imagerie révélait une masse tissulaire sur l’isthme d’un rein en fer à cheval, classée Bosniak IV. Une isthmectomie par laparotomie médiane fut réalisée. L’examen anatomopathologique confirmait une inflammation chronique avec deux abcès.
Conclusion : ce cas illustre une localisation atypique d’abcès rénaux sur rein en fer à cheval, posant un défi diagnostique et thérapeutique.

SUMMARY
Introduction: horseshoe kidney is a congenital anomaly resulting from the fusion of the kidneys. Although frequently asymptomatic, it may predispose to various urological complications. We present a rare case of a renal abscess localized at the isthmus of a horseshoe kidney
Case report: a 42-year-old woman presented with chronic right hypochondrium pain. Imaging revealed a tissue mass on the isthmus of a horseshoe kidney, classified as Bosniak IV. A midline laparotomy with isthmectomy was performed. Histopathology confirmed chronic inflammation with two abscesses.
Conclusion: this case highlights an unusual location of renal abscesses on a horseshoe kidney, raising diagnostic and therapeutic challenges.
