RESUME
Introduction : l’avulsion du globe oculaire est le déplacement de celui-ci en dehors de la cavité orbitaire. Les auteurs rapportent 3 cas d’avulsion post-traumatique du globe oculaire pris en charge à l’HIA OBO de Libreville.
Observation 1 : une patiente de 5 ans, était adressée pour traumatisme oculopalpébral droit survenu lors d’un accident domestique. L’examen notait des plaies palpébrales et un déplacement du globe oculaire hors de l’orbite. Le diagnostic d’avulsion du globe avait été posé. Le globe oculaire avait été réintégré dans l’orbite.
Observation 2 : une patiente de 29 ans, était adressée pour un traumatisme oculo-orbitaire droit lors d’un accident de la voie publique. L’examen notait une absence de perception lumineuse et une exotropie. La TDM avait objectivé une section du nerf optique, posant le diagnostic d’avulsion du globe. La patiente avait bénéficié d’une réintégration du globe oculaire.
Observation 3 : un patient de 55 ans, avait consulté pour un traumatisme de l’oeil gauche, survenu dans les circonstances de rixe. A l’examen, on notait une absence de perception lumineuse et un déplacement du globe oculaire en avant de l’orbite avec désinsertion conjonctivale. Le diagnostic d’une avulsion du globe était posé. Le patient avait été pris en charge avec réintégration du globe oculaire.
Conclusion : l’avulsion post-traumatique du globe est accident grave qui engage le pronostic anatomique et fonctionnel du globe oculaire. Le rôle du chirurgien sera de sauver le pronostic anatomique de l’organe.

SUMMARY
Introduction: the avulsion of the eyeball is the displacement of it outside the orbital cavity. The authors report 3 cases of post-traumatic avulsion of the eyeball treated at the HIA OBO in Libreville.
Observation 1: a 5-year-old patient was referred for right oculopalpebral trauma that occurred during a domestic accident. The examination noted palpebral wounds and displacement of the eyeball out of the orbit. The diagnosis of globe avulsion was made. The eyeball had been reintegrated into the socket.
Observation 2: a 29-year-old patient was referred for right oculo-orbital trauma following a road accident. The examination noted an absence of light perception and exotropia. The CT had objectified a section of the optic nerve, making the diagnosis of avulsion of the globe. The patient had benefited from a reintegration of the eyeball.
Observation 3: a 55-year-old patient had consulted for trauma to the left eye, which occurred in the circumstances of a fight. On examination, there was an absence of light perception and a displacement of the eyeball in front of the orbit with conjunctival disinsertion. The diagnosis of globe avulsion was made.
The patient had been managed with reintegration of the eyeball.
Conclusion: post-traumatic avulsion of the globe is a serious accident that involves the anatomical and functional prognosis of the eyeball. The role of the surgeon will be to save the anatomical prognosis of the organ.
