RESUME
Introduction : les auteurs rapportent l’observation clinique d’un cas grave de contusion abdominale avec pour objectif de ressortir les difficultés diagnostiques et les résultats de la prise en charge en milieu défavorisé.
Observation : une patiente de 24 ans a été admise pour une contusion de l’abdomen par accident de la circulation. Cinq heures après son admission elle présentait un état de choc hémorragique par rupture de rate avec découverte peropératoire d’une perforation gastrique. Une fistule stercorale a été constatée au 5eme jour post opératoire. La relaparotomie révélait une perforation négligée du colon transverse et du grêle. Sept jours après la fermeture des perforations, apparaissaient une hématémèse massive et mortelle.
Conclusion : toute contusion abdominale mérite une surveillance et une attention particulière. La prise en charge doit être rapide, effective et multidisciplinaire.

SUMMARY
Introduction : The authors report the clinical observation of a serous case of abdominal contusion with the aim of
highlighting the diagnostic difficulties and the results of the treatment.
Case report : a 24 year old patient was admitted for an abdominal contusion by road accident . Five hours after admission she had a state of hemorrhagic shock by rupture of the spleen in intra operative discovery of gastric perforation. A stercoral fistula was observed on the 5th post operative day. The relaparotomy revealed a neglected perforation of the transverse colon and hail. Seven day after the closure of the perforation a massive and deadly hematemesis appeared.
Conclusion : any abdominal contusion deserves special special attention and attention. The care most be fast, effective and multidisciplinary.
